With 50% of Australians aged 45-64 affected by one or more chronic conditions, our independent doctors offer personalised consultations to create tailored GP Management Plans (GPMP) and Team Care Arrangements (TCA) for you or your loved ones.

Helps manage long-term conditions. Medicare covers Allied Health visits. Evidence-based care plans like Team Care Arrangements are designed to support better health outcomes and overall well-being.
If you meet the criteria, our nurses will help you develop your GPMP and TCA, including an Allied Health Referral. The process takes about 30 minutes, and referrals will be arranged as part of the process. Sessions for completing your GPMP and TCA are free for eligible patients with a valid Medicare card.
Eligible patients can access up to five subsidised Allied Health consultations per year. New plans become available after 12 months. Our independent doctors are here to guide you toward better health and effective chronic illness management.
