Chronic Disease Management

Chronic diseases are a leading cause of death and disability in Australia, with over half of Australians at risk due to lifestyle factors.

At Branxton Healthcare, we are dedicated to supporting individuals through effective Chronic Disease Management. With 50% of Australians aged 45-64 affected by one or more chronic conditions, our independent doctors offer personalized consultations to create tailored GP Management Plans (GPMP) and Team Care Arrangements (TCA) for you or your loved ones.

Chronic Disease Management Plans

At Branxton Healthcare, we offer two kinds of chronic disease management plans to support high‑quality, long‑term care for your health.

A GP Management Plan

This is a personalized care plan tailored to help you manage chronic diseases. Following a consultation with one of our independent GPs, your GP will consider your medical, physical, psychological, and social needs to create a comprehensive management plan. The GP Management Plan is reviewed every six months or as needed if there are significant changes in your condition. Branxton Healthcare will notify you when your plan needs renewal.

A Team Care Arrangement

In conjunction with your GP Management Plan, a Team Care Arrangement helps manage long-term conditions. This arrangement focuses on the root causes of chronic diseases. Through this plan, Medicare will cover the costs of Allied Health visits, supporting a holistic approach to health. Evidence‑based care plans like Team Care Arrangements are designed to support better health outcomes and overall well‑being.

Eligible Patients

The Key to Better Health

If you meet the criteria, our nurses will help you develop your GPMP and TCA, including an Allied Health Referral. The process takes about 30 minutes, and you’ll receive your referral promptly. Sessions for completing your GPMP and TCA are free for eligible patients with a valid Medicare card.

Consistent Care and Ease

Eligible patients can access up to five subsidised Allied Health consultations per year. New plans become available after 12 months. Our independent doctors are here to guide you toward better health and effective chronic illness management.


Urgent Medical Care

Our experienced doctors assess and manage a range of acute conditions and can advise on appropriate next steps, including referral to emergency services when required. For serious or life-threatening conditions, patients should contact emergency services or attend the nearest hospital emergency department.

Comprehensive Care When You Need It

We provide medical care for urgent, non-life-threatening health concerns during our clinic operating hours. Our doctors assess acute conditions and advise on appropriate care, including referral to hospital emergency services when required.
